By clicking on the "Register" button, I certify that I am at least l8 years of age and authorized by the telephone account owner to make changes to and incur changes on this telephone account provided. By completing and submitting this Letter of Agency Application (LOA), I am authorizing Netcom Systems Group, LLC to become my new long distance service provider for the provision of long distance service for the telephone number (s) listed above. I authorize Netcom Systems Group to act as my agent to make this change occur, and direct my current provider to assist Netcom Systems Group in effectuating the change. If I later wish to return to my previous long distance provider, I may be required to pay a reconnection charge to that company. I also understand that my new long distance service provider may have different calling areas, rates and charges than my current long distance provider and that by completing this form, I indicate that I understand those differences (if any) and am willing to be billed accordingly. I further understand that only one carrier may be designated as my preferred state-to-state and instate toll carrier (interstate and intralata) and hereby appoint Netcom Systems Group to be my carrier for both interstate and Intralata.
